I accidentally sent an email to a test distribution list (which was quite obviously a test, based on the content of the email). However, my boss demanded that I reply all with an apology, which ONLY THEN called attention to it and resulted in a bunch of other replies. People had correctly ignored the first one. Because grownups.

The reply all button should be hidden inside a drop down menu.

My department does this all the time, replying to all when they should reply to the sender only.
“All course directors please please report your [insert whatever it is here] to the department office.” Then everyone replies all 20 times over with stuff I don’t need to read. Grrr.
And what’s with this “so and so would like to recall the message...”. Too late, I already read it. It can’t be recalled!

One of my workplaces had a particularly ice cold treatment of email blasts. At some point, it became a “thing” for people who were leaving to send out blasts to all 2500 employees whenever they left their jobs/got fired. Two reply alls I remember most were:
“I’ve never met you, and I don’t know who you are, but I’m sure that your highly personal approach to interpersonal communication will serve you well in your future pursuits.”
And another, coming from a particularly reviled department was met with
“I have no idea who you are, but I’m glad you’re leaving.”

We had a massive email slow-down last year because somebody sent an email to everybody in the company by accident and everybody told him not to do that...via the reply all button. It was a never-ending chain built from the finest irony.
